Signature Release - Authorize To Leave At Door

To authorize Federal Express to leave a package at your door, download and print a release form below. The form must be available to the FedEx driver at the delivery address with an authorized signature. By signing either form, you agree that Motorcycle USA and FedEx will not be liable for any loss or damage from leaving a shipment. You will need Adobe Acrobat to view and print the forms.
